Many dedicated CUPE 503 Members are front line workers in the fight against Covid-19. If you believe you may have been exposed to Covid-19 in the course of your employment, it is important to document and report this potential exposure to the WSIB by completing the WSIB Worker’s Exposure Incident Form. CUPE 503 recommends that Workers send their completed forms directly to the WSIB, and then provide a copy to their Employer. Completed forms can be faxed to 1-888-313-7373, or can be uploaded to their website at: https://www.wsib.ca/en

Should a worker experience any illness in relation to an exposure incident, then please complete a Worker’s Report of Injury/Disease (Form 6) as soon as possible.
